When is it necessary to require that winners notarize and return an affidavit of eligibility?

Sorry for the late reply. Whether to use an affidavit is totally up to the sponsor. The affidavit just proves to the sponsor that the winner is who they say they. If a prize is over $600, sponsors usually send winners a 1099. In that case, the affidavit shows the SSN number so the sponsor can match the SSN to the 1099 form for their records.
It's not legally required to use an affidavit.
